Adult & Child Case Management

Carlton, Lake, Cook and St. Louis Counties contract with HDC to provide case management services to adults and children with serious mental health issues. Case managers work closely with individuals and families to help them access needed supports and services to maximize independence, mental health, and community integration.

Brighter Futures

The Brighter Futures Initiative provides early intervention and prevention services to children ages 8-12 who are not receiving mental health services, but have been identified by teachers or others as being at risk due to concerns such as ; school issues, family issues, community issues such as drug/alcohol/tobacco use, violence, pre-delinquency issues, as well as others that affect youths. The Brighter Futures Initiative is not a mental health treatment program. It is an early intervention and prevention resource that works with families to increase academic, social, and /or parenting skills.

Children’s Therapeutic Services & Supports

CTSS is designed to help children under the age of 18 with mental health, emotional and/or behavioral issues. This service is based on the unique and changing needs of the child and is designed to improve the child’s functioning within the family and community. CTSS activities take place in the child’s home and the community and referrals for services come from County Social Services. CTSS is available in St. Louis and Carlton Counties.

Community Wraparound/Coordinated Service Team

Community Wraparound provides families the opportunity to create their own plan of care through teamwork with professionals and others. The purpose of Community Wraparound is to bring families together with friends, family members, professionals, and others to develop a plan of care that is guided by the family with support from others. This is a FREE service that provides your family with the opportunity to meet once a month with individuals such as friends, family members, and service providers, who have a direct impact on you and your family’s life. This group forms what is known as your Wraparound Team. The team then supports your family and assists in meeting identified needs and accomplishing family goals. CSP & ARMHS

The goal of both CSP and ARMHS is to assist persons with mental illness to live independently and successfully in the community. HDC provides supportive counseling, independent living and socialization skills training, and advocacy to resolve problems. CSP services are funded through state/county grants for clients with a designation of SMPI.

ARMHS services are funded by Medical Assistance and PMAP Health Plans. These programs are located at:

Dialectical Behavior Therapy (DBT)

DBT is a cognitive-behavior treatment model developed specifically for treating individuals who meet criteria for Borderline Personality Disorder. Using the DBT model, individuals learn to regulate their emotions and reactions, learn self-acceptance, and learn how to positively influence their environment. Group therapy is available at 1401 East First Street, Duluth, MN office only.

Duluth ACT Program

Assertive Community Treatment (ACT) teams provide the most intensive community services available. Services are provided to clients with a SMI or SPMI diagnosis who are referred by the St. Louis County Social Services mental health unit. Staff to client ratio is 1/10, services are available 7 days a week, and include but are not limited to, medication management, psychiatry, case management, substance abuse treatment, crisis intervention, employment support, independent living skills training, and housing support. Services are based on individual needs of the client and provided primarily in the client’s home. The Duluth ACT program is located at 1026 East Superior Street, Duluth, MN.

Eating Disorders Treatment

Eating disorders are highly complex, bio-psychosocial problems which require specialized care for effective treatment. The Human Development Center’s Eating Disorders Treatment Program, led by Dr. Carolyn Phelps, provides expert specialized care to people of all ages suffering from Anorexia Nervosa, Bulimia Nervosa, and Binge Eating/Compulsive Overeating, regardless of gender. We provide help to females and males of all ages, who:

  • Self starve
  • Diet constantly
  • Purge, vomit, use laxatives or diet pills
  • Binge eat or eat compulsively
  • Over-exercise
  • Over-train
  • Have body image problems
  • Think obsessively or excessively about food, weight, shape and/or physical appearance
  • Are experiencing problems after bariatric surgery

We also address the special needs of eating disordered athletes, dancers, and patients with co-existing and co-complicating medical (e.g., diabetes) and psychological (e.g., depression, anxiety) problems.

The Eating Disorders Treatment Program offers the following services:

Employment Connection Program

Our experience is that persons with mental illness prefer to work and have the ability to do so, but often need advocacy and support to find and keep a job. We provide services in St. Louis and Lake County in two distinct ways:

  1. Transitional Employment opportunities that help develop employment skills (i.e. attendance issues, attitude issues, benefits issues, etc.) for clients who have been unsuccessful in the work place, or have not worked. These work opportunities take place on either our Janitorial Crew or a Moving Crew. Clients gain work experience while being supervised by program staff.
  2. Community Placement services that assist clients to define vocational goals, and then find and maintain competitive employment in the community.

Forensic Evaluations

Through its Center for Forensic Mental Health, HDC offers a variety of court-ordered forensic evaluations, including evaluations of Competency to Stand Trial, Criminal Responsibility, Juvenile Certification Evaluations, Civil Commitment, CHIPS Parenting, Pre-Sentencing and psychosexual evaluations. In addition, the Center offers Pre-Employment/Fitness for Duty Evaluations and offender treatment. Evaluations are available for all counties. Harmony Club

Harmony Club offers persons with mental health disabilities the opportunity to be part of a social club, making friends and learning new skills. Harmony uses a consumer empowerment model, integrating consumer-led activities with the more traditional staff leadership. While Harmony is not a treatment program, we do work closely with staff from other HDC CSP services to assist consumers with vocational, housing, and psychotherapy needs.

Harmony Club operates out of the 1730 E. Superior Street building in Duluth.

Homeless Project

We believe that homelessness is not a choice, and that given the opportunity for safe, affordable housing, everyone prefers a permanent place to live. Our Homeless Project serves individuals in Duluth who are homeless and have a mental illness. Many of those we serve also have health problems and substance abuse issues. We provide community outreach to locate homeless persons, and assist them to obtain and keep permanent housing. We also provide case management, employment assistance, transportation, benefits assistance, and crisis intervention. We work closely with other service providers, including St. Louis County Public Health and Human Services, Duluth Police Department, CHUM, The Union Gospel Mission, Damiano Center, Salvation Army, WCDO, Housing Access Center, Life House, AICHO, Center for Alcohol and Drug Treatment, and Northeast Regional Correction Center. The Homeless Project is located at 315 N. 2nd Avenue West, Duluth, MN.

Intensive In-Home Program

In-Home services are designed for families where a child’s problems interfere significantly with school performance and/or success in the community. Our program focuses on the strengths and abilities of the family. Intensive In-Home services are designed to present a non-threatening approach to the family, lower stress and family conflict, reduce the chance of out-of-home placement whenever possible and appropriate, provide skills training such as anger control, parenting techniques, conflict resolution, stress management, and communication, educate the family the regarding child’s mental health issues, and link family to community services. Families who qualify for the program must meet the following criteria:

  • Must be receiving Medical Assistance. Cost is $0 to the family.
  • Must be willing to allow the team to visit the home for up to 6-8 hours per week.

Jill's Place

The mission of Jill’s place is to provide a comfortable and safe setting for people experiencing mental illness to connect with their peers, socialize, volunteer, and enjoy recreation. Our comfortable space has a Wii System, DVD player, many games, music, snacks, magazines and more, all for the comfort of our guests. Memory Assessment

We provide specialized assessment of memory problems, including possible dementia. Patients are seen by our collaborative team of Geriatric Nurse Practitioner and Geriatric-Neuropsychiatric Psychiatrist. Additional assessment by a psychologist is available as needed. Nursing Home and Neuropsychiatry

Our older adult team visits a large number of nursing homes in Carlton, St. Louis, and Lake Counties, as well as assisted living settings and brain injury programs. We also provide specialized consultations for adults with adjustment problems connected with neurological disorders such as Multiple Sclerosis, Parkinson’s Disease, Epilepsy, brain injury, and developmental disability.

Outpatient Chemical Dependency

HDC offers Adult Chemical Dependency Outpatient Treatment in Douglas County. Clients can choose between group therapy, support groups, women’s recovery groups, or individual sessions. Counselors are trained in different modalities of treatment such as twelve step/disease model and cognitive behavioral concepts.

The Outpatient Chemical Dependency Program works closely with the psychiatry and mental health professionals at HDC to ensure that clients are receiving the all inclusive care that is necessary in each individual case.

HDC also offers Adolescent Chemical Dependency Treatment on an outpatient basis. Adolescents struggling with drug and/or alcohol abuse have education, treatment and individual sessions with trained counselors available to them. The adolescent treatment program focuses on the many complex reasons that teens experiment with drugs rather than using the “just say no” adage.

Outpatient Consultations and Neuropsychiatry

We provide consultations and ongoing services for older adults with depression, anxiety, and other mental health problems associated with aging and physical health problems. Initial evaluations are typically done by a Geriatric Nurse Practitioner with psychiatric expertise, in collaboration with our Geriatric Psychiatrist. Outreach Center

The Outreach Center is a consumer-run drop-in center located at 24 10th Street in Cloquet, MN. The Outreach Center offers Supportive and Recreational Services, such as Information/Education, Recreational and Social Events, Bipolar Support Group, Friends and Family Bipolar Support Group, BINGO, Crafts, Employment and Volunteer Opportunities, and Meals Twice a Week. Everyone is welcome and it is open Monday through Saturday 10:00am to 4:00pm. Outreach Center Apartments

Sometimes living with a mental illness makes maintaining independent living a difficult thing to do. Residents of the Outreach Center Apartments in downtown Cloquet, MN will have available to them an array of services designed to help them succeed in their apartment and maximize their potential in the community. Services available include Housing Assistance, Medication Monitoring, Employability Support, Outreach, Crisis Assistance, Education and Support, Assistance with Activities of Daily Living. People eligible for the apartments must meet certain criteria. They include meeting HUD’s definition of homeless, being disabled, having a mental illness and meeting income eligibility.

Project Reach Out

Project Reach Out works to improve the network of support to both youth and their families through confidential and voluntary crisis intervention referral service. Each year, many Wisconsin families are confronted with a crisis such as a teen runaway. Teens run for a variety of reasons. These reasons can include; unresolved conflict, lack of communication and differences over roles, rules, and expectatations. Project Reach Out can help you during these difficult times. This program’s goals are to alleviate the problems of runaway and homeless youth by insuring that each community has access to 24 hour crisis intervention services; to reunite youth with their families when appropriate, and encourage the resolution of inter-family problems through mediation and other services; to strengthen family relationships and encourage stable living conditions for youth; and to help youth decide upon a constructive course of action and constructive decision making in a nonjudgmental atmosphere. Psychiatry

These services are provided at all HDC locations and include evaluations, treatment, and medication management of depression, anxiety, and other psychiatric disorders. Our staff of psychiatrists and psychiatric nurses are skilled in pharmacology for children and adults.

Services are offered at our offices as well as several contracted locations, to include nursing homes, assisted living facilities, brain injury programs, juvenile centers, children’s residential treatment programs, and correctional facilities. Psychiatry also has services available via ITV to Cook County, St. Peter, and other locations. Most major insurance plans are accepted; we also have a sliding fee scale for uninsured clients.

Current access is limited to clients receiving other service(s) at HDC.

Psychological Testing & Assessments

Services performed at HDC include psychological evaluations and assessments with children, adolescents, adults and seniors. These evaluations and assessments are conducted under the direction of a licensed psychologist and may include personality, behavior and/or intelligence testing.

Psychotherapy

HDC offers therapy in individual, family and group settings. Some of the problems addressed by HDC’s therapists include depression, anxiety, grief, abuse, life stress, family and marital issues, pain management, eating disorders and anger management. Services are open to children, adolescents & adults. Most major insurance plans are accepted and sliding fee is available with eligibility requirements for uninsured clients. Psychotherapy is available in our Cloquet, Duluth, Grand Marais, Moose Lake, Spirit Valley, Superior, and Two Harbors offices.

Residential Chemical Dependency Services

HDC Chemical Dependency Services offers an inclusive approach to treating substance dependency and co-occurring mental health disorders. Our services include individualized treatment plans, off site mental health services, structured programming, and case management. We are HFS 83 certified in accordance with the Department of Health and Family Services.

Community Based Residential Facility

Our Community Based Residential Facility is located in Superior, WI. It is close to parks, medical facilities, and other community resources. The unit is unlocked and staffed twenty-four hours per day by qualified and compassionate individuals. Two certified chemical dependency counselors primarily oversee daytime programming, while residential counseling assistants supervise evenings and weekends.

This twenty bed co-ed facility utilizes several modalities in the treatment of addiction such as the Matrix Model, Twelve Step, Cognitive/Behavioral, and Individual Counseling. In addition, a variety of specialized subject groups are held weekly such as men’s/women’s issues, grief/coping, dual diagnosis, art therapy, nutrition, and family education. Lectures and education are provided on an ongoing rotating schedule by a diverse group of professionals from the agency and surrounding area.

Transitional Programming

The average stay in our residential program is approximately 30 days. After a period of treatment, a person may enter into the agency’s transitional program. This program is offered on a case-by-case basis to those individuals looking to seek housing, employment, and external supports prior to discharge. Persons allowed to participate in the transitional program will have demonstrated a great desire to change as well as the motivation to continue seeking recovery while tending to other areas of their lives.

Safe Schools | Healthy Students

The Duluth, Proctor, and Hermantown school districts joined together in obtaining Federal grant funding for the “Safe Schools, Healthy Students” program. This program has a number of “elements” all targeted at providing a safe and healthy environment in our schools. HDC works cooperatively with all three school districts to provide therapy/counseling at all school sites, and at HDC clinic locations.. Referrals come from school personnel at each location.

Strong Families

The Strong Families program provides the same services as CTSS, however all children and their families can be served regardless of their insurance status because the program is partially grant funded. In order to receive these services a child must have a mental health diagnosis given within six months of the time of referral. These services also take place in the child’s home and community. Referrals come from County Social Services, Probation, Therapists and self-referral.

Strong Families is available in the Duluth area This program is housed in the Spirit Valley location.

T-ACT

T-ACT is a program designed for young adults, ages 18 to 25 years, who have serious mental health issues and are homeless, at risk, or otherwise disenfranchised. An assertive outreach approach assists clients with life skills, social skills, housing, jobs, med management, and other basic needs. The program is collaboratively staffed by St. Louis County Social Services, HDC, and Life House.
